However, their efficacy is reduced during inflammation as well as due PR:PROJECT_SUMMARY to platelet storage lesion, including metabolomic shifts and changes in surface PR:PROJECT_SUMMARY markers of stored PCs. This study aims to identify disparities between PR:PROJECT_SUMMARY short-term and long-term stored PCs during controlled inflammation, focusing on PR:PROJECT_SUMMARY distinct metabolic pathways, alterations in surface markers and post-transfusion PR:PROJECT_SUMMARY recovery (PTR). Twenty-four male subjects received lipopolysaccharide (LPS) or PR:PROJECT_SUMMARY saline as control following an autologous transfusion of either short-term (2 PR:PROJECT_SUMMARY days stored) or long-term stored (7 days stored) PCs. Metabolomics and surface PR:PROJECT_SUMMARY markers of these transfused PCs were assessed before transfusion using mass PR:PROJECT_SUMMARY spectrometry and flow cytometry, respectively. Biotin-labeled platelets were PR:PROJECT_SUMMARY used to assess surface markers after transfusion and determine PTR. Prior to PR:PROJECT_SUMMARY transfusion, short-term stored PCs demonstrated increased glycolysis, pentose PR:PROJECT_SUMMARY phosphate pathway activity, dense granule components (e.g., serotonine, PR:PROJECT_SUMMARY adenosine diphosphate, epinephrine), and purine, arginine, and tryptophan PR:PROJECT_SUMMARY metabolism. In contrast, long-term stored PCs exhibited elevated PR:PROJECT_SUMMARY transsulfuration and taurine metabolism, along with higher levels of CD62P and PR:PROJECT_SUMMARY CD63. During inflammation, a decreased PTR was found, particularly in long-term PR:PROJECT_SUMMARY stored PCs. Higher expression of signaling molecules and lower CD62P and lactate PR:PROJECT_SUMMARY levels were correlated with improved PTR. Differences in metabolic pathways, PR:PROJECT_SUMMARY surface markers, and PTR were identified between short-term and long-term stored PR:PROJECT_SUMMARY PCs in a controlled human experiment – suggesting a preference for the use of PR:PROJECT_SUMMARY short-term stored PCs during inflammation. study on the influence of CO:COLLECTION_SUMMARY storage time of platelet concentrates in a two-hit healthy volunteer model on CO:COLLECTION_SUMMARY transfusion-related acute lung injury; the results of this study must therefore CO:COLLECTION_SUMMARY be considered exploratory [Transfusion. 2022;62(12):2490-2501.]. The sample size CO:COLLECTION_SUMMARY was determined based on the primary outcome of the overall study, CO:COLLECTION_SUMMARY bronchoalveolar lavage fluid protein content, as previously described CO:COLLECTION_SUMMARY [Transfusion. 2022;62(12):2490-2501]. Male subjects, aged 18-35 years, were CO:COLLECTION_SUMMARY allocated to participate in this part of the open-label randomized controlled CO:COLLECTION_SUMMARY study. Subjects underwent a pre-participation screening process, which included CO:COLLECTION_SUMMARY a review of their medical history, assessment of vital signs, physical CO:COLLECTION_SUMMARY examinations, and laboratory tests encompassing blood and urine analysis. CO:COLLECTION_SUMMARY Subjects were prohibited from engaging in any other intervention trials both CO:COLLECTION_SUMMARY three months before and after the study. Prior to enrollment, all volunteers CO:COLLECTION_SUMMARY provided written informed consent. In our current analysis, solely subjects CO:COLLECTION_SUMMARY (n=24) who received a platelet transfusion were included. The study procedures CO:COLLECTION_SUMMARY have been reviewed and approved by the Amsterdam UMC Medical Ethical Committee CO:COLLECTION_SUMMARY (2014_294#B2014961) and are according to the principles of the Declaration of CO:COLLECTION_SUMMARY Helsinki and Good Clinical Practice (GCP). The study is registered at the CO:COLLECTION_SUMMARY International Clinical Trials Registry Platform (ICTRP): NL-OMON26852. CO:COLLECTION_SUMMARY Randomization and blood donation The first 12 subjects (part I) were allocated CO:COLLECTION_SUMMARY to receive Escherichia coli (O113) lipopolysaccharide (LPS; dose 2 ng/kg; lot CO:COLLECTION_SUMMARY number 94332B1) to incite an inflammatory condition through toll-like receptor 4 CO:COLLECTION_SUMMARY stimulation, whereas the last 12 subjects (part II) received an equal volume of CO:COLLECTION_SUMMARY physiological saline (0.9% [w/v] sodium chloride) as control [Blood. CO:COLLECTION_SUMMARY 2006;107(2):637-641 and Platelets 2016;27(5):479-483.]. Subjects were scheduled CO:COLLECTION_SUMMARY to participate twice, once with and once without LPS, with a minimum interval of CO:COLLECTION_SUMMARY one year between sessions. Subjects were randomly assigned to receive one of the CO:COLLECTION_SUMMARY following: short-term stored autologous PCs (2 days stored) or long-term stored CO:COLLECTION_SUMMARY autologous PCs (7 days stored). Treatment allocation concealment was achieved by CO:COLLECTION_SUMMARY generating a randomization sequence using R (version 4.3.2, Boston, MA, USA). CO:COLLECTION_SUMMARY The sequence was placed in consecutively numbered opaque envelopes, which were CO:COLLECTION_SUMMARY opened only after each subject's inclusion. Depending on group allocation, the CO:COLLECTION_SUMMARY subjects donated 1 unit (± 300 mL) of apheresis platelets 2 or 7 days before CO:COLLECTION_SUMMARY the study day at Sanquin Blood Bank. Apheresis PCs were obtained and stored in CO:COLLECTION_SUMMARY compliance with Dutch Blood Bank standards, and preserved in 100% platelet CO:COLLECTION_SUMMARY additive solution E (PAS-E). The trial was conducted as an open-label randomized CO:COLLECTION_SUMMARY study, as subjects were required to donate either 2 or 7 days before the CO:COLLECTION_SUMMARY experiment. CO:SAMPLE_TYPE Platelets #TREATMENT TR:TREATMENT_SUMMARY Biotinylation of platelets: On the day of the transfusion, platelets were TR:TREATMENT_SUMMARY biotinylated according to GCP as previously described [Transfusion. TR:TREATMENT_SUMMARY 2019;59(9):2964-2973.]. In total, 50 mL of the platelet concentrate was TR:TREATMENT_SUMMARY biotinylated with 5 µg/mL of NHS-biotin (EZ-Link™, Thermo Fisher Scientific, TR:TREATMENT_SUMMARY Waltham, MA) and returned to the storage bag. Experiment: Subjects were admitted TR:TREATMENT_SUMMARY for one day to the research unit of the department of intensive care of the TR:TREATMENT_SUMMARY Amsterdam University Medical Center, the Netherlands. Before the study TR:TREATMENT_SUMMARY procedures, a research physician assessed the subjects, and blood was cross TR:TREATMENT_SUMMARY matched as an additional safety procedure. A peripheral venous catheter (18 G; TR:TREATMENT_SUMMARY BD, Plymouth, United Kingdom) was inserted to administer 1L of normal saline for TR:TREATMENT_SUMMARY prehydration one hour before the experiment began. Maintenance fluids were then TR:TREATMENT_SUMMARY set at 1.5L of physiological saline per 24 hours until the conclusion of the TR:TREATMENT_SUMMARY experiment. An arterial catheter (20 G; BD, Plymouth, United Kingdom) was TR:TREATMENT_SUMMARY inserted using ultrasound into the radial artery. During admission, subjects TR:TREATMENT_SUMMARY underwent continuous monitoring, including blood pressure, temperature, heart TR:TREATMENT_SUMMARY rate, and respiratory rate. Blood samples were collected from the platelet TR:TREATMENT_SUMMARY concentrate (PC) storage bag for further analyses as described below. Following TR:TREATMENT_SUMMARY the completion of safety checks and sample collection, subjects were TR:TREATMENT_SUMMARY administered either LPS or control. Two hours afterwards, a 300mL autologous PC TR:TREATMENT_SUMMARY (including 50 mL biotinylated platelets) was infused over exactly 50 minutes TR:TREATMENT_SUMMARY before the infusion set was locked and the post-transfusion period commenced. TR:TREATMENT_SUMMARY Ten minutes, 30 minutes, 1, 2, 3, 4, and 6 hours post-transfusion, blood samples TR:TREATMENT_SUMMARY were drawn from an arterial line and further processed and analyzed. TR:TREATMENT_SUMMARY Additionally, platelet counts were measured before and after transfusion using a TR:TREATMENT_SUMMARY hematology analyzer (XN-9000, Sysmex, Norderstedt, Germany). The platelet TR:TREATMENT_SUMMARY increment was determined by the difference in platelet count before and 1 hour TR:TREATMENT_SUMMARY after the transfusion, and the corrected count increment (CCI) was calculated by TR:TREATMENT_SUMMARY platelet count increment per µL × body surface area (m²) / unit content. On a TR:TREATMENT_SUMMARY return visit, 48 hours post-transfusion, the last blood samples were collected TR:TREATMENT_SUMMARY using venipuncture.
