Current therapies PR:PROJECT_SUMMARY including drugs targeting BRAF and MEK significantly improve the prognosis of PR:PROJECT_SUMMARY metastatic melanoma patients, yet innate or acquired resistance challenges PR:PROJECT_SUMMARY long-term responses. We have shown previously that fatty acid beta-oxidation PR:PROJECT_SUMMARY (FAO) is up-regulated during the acquisition of BRAF-inhibitor (BRAFi) PR:PROJECT_SUMMARY resistance and that the FDA approved drug ranolazine, by targeting FAO PR:PROJECT_SUMMARY attenuates the development of acquired resistance. However, how PR:PROJECT_SUMMARY ranolazine-induced metabolic rewiring increases cell death is unclear. Here we PR:PROJECT_SUMMARY identify ranolazine as a ferroptosis inducer in BRAFi-resistant melanoma, in PR:PROJECT_SUMMARY which FAO serves as a ferroptosis surveillance mechanism. Accordingly, in PR:PROJECT_SUMMARY progressed tumours of BRAFi treated patients up-regulation of FAO regulators PR:PROJECT_SUMMARY correlates with increased expression of ferroptosis markers. BRAFi resistant PR:PROJECT_SUMMARY cells are heavily poised for execution of ferroptosis; they display reduced PR:PROJECT_SUMMARY glutathione levels, higher levels of long-chain polyunsaturated fatty acid PR:PROJECT_SUMMARY (PUFA) membrane-incorporation, and increased membrane-resident phospholipid PR:PROJECT_SUMMARY oxidation, all of which is amplified by ranolazine. Counteracting ranolazine PR:PROJECT_SUMMARY action is MBOAT1/2 mediated phospholipid remodeling, which initiates reduced PR:PROJECT_SUMMARY PUFA membrane-incorporation as ferroptosis surveillance mechanism. We show that PR:PROJECT_SUMMARY the androgen receptor (AR), which is a determinant of BRAFi resistance, controls PR:PROJECT_SUMMARY MBOAT1/2 expression, thereby contributing to ferroptosis resistance. In BRAFi PR:PROJECT_SUMMARY resistant tumours and cell lines, we confirm AR upregulation predominantly in PR:PROJECT_SUMMARY the MITFlow/AXLhigh undifferentiated/neural-crest like state, but it also occurs PR:PROJECT_SUMMARY in the MITFhigh/AXLlow differentiated melanocytic state. The AR antagonist PR:PROJECT_SUMMARY enzalutamide sensitizes AR expressing melanoma cells to RSL3 and erastin PR:PROJECT_SUMMARY independent of phenotype state, but in FAOhigh BRAFi relapsed tumours AR PR:PROJECT_SUMMARY up-regulation correlates with the undifferentiated/neural-crest like (UD/NC) PR:PROJECT_SUMMARY state, and enzalutamide synergizes with ranolazine in ferroptosis-induction in PR:PROJECT_SUMMARY UD/NC cells. Thus, therapeutically combining ranolazine with the AR inhibitor PR:PROJECT_SUMMARY enzalutamide to induce ferroptosis can circumvent dedifferentiation related PR:PROJECT_SUMMARY BRAFi resistance and could increase therapeutic activity and long-term efficacy.
